Terms of Service Agreement

By using this website, booking and receiving services from Kodama Wellness, PLLC, you acknowledge and agree to the following:

Scope of Practice & Nature of Services

Massage and bodywork services provided are therapeutic in nature and are intended to support relaxation, mobility, and overall well-being.

These services are not a substitute for medical examination, diagnosis, or treatment. The practitioner does not diagnose conditions, prescribe treatment, or perform spinal or skeletal adjustments.

You understand that it is your responsibility to seek care from a qualified healthcare provider for any medical concerns.

Client Responsibility & Communication

You agree to:

• Provide accurate and complete health information to the best of your knowledge

• Inform the practitioner of any injuries, conditions, or changes in your health

• Communicate immediately if you experience pain, discomfort, or need adjustments during the session

Failure to disclose relevant health information releases the practitioner from liability related to those conditions.

Consent to Receive Bodywork

You voluntarily consent to receive massage and bodywork services and understand that:

• Techniques may include therapeutic touch, movement-based bodywork, and pressure applied to soft tissue

• Participation (active or passive movement) may be incorporated when appropriate

You may request modification or stop the session at any time.

Professional Boundaries & Conduct

Kodama Wellness maintains a professional, therapeutic environment at all times.

• Any illicit, inappropriate, or sexually suggestive remarks or behavior will result in immediate termination of the session

• Full payment for the scheduled service will still be required

• Future services may be declined at the practitioner’s discretion

Appointment Timing & Late Arrival

Sessions begin and end as scheduled.

• If you arrive late, your session may be shortened to respect the next client

• Full session fees still apply

• Arrivals more than 15 minutes late may be treated as a missed appointment

Cancellation & Rescheduling Policy

Your appointment time is reserved exclusively for you.

• A minimum of 24 hours’ notice is required to cancel or reschedule

• Late cancellations (less than 24 hours) may be charged up to 100% of the session fee

• No-shows will be charged the full session fee

Illness & Health Considerations

To protect both client and practitioner:

• Please reschedule if you are experiencing symptoms of illness (fever, cough, flu-like symptoms, contagious conditions)

• If illness is disclosed at the time of appointment, the session may be declined and treated as a late cancellation

Payment & Responsibility

Payment is due at the time of booking or as otherwise agreed.

You are responsible for all fees associated with scheduled services, including late cancellations or missed appointments.

By using this website, booking, and receiving services from Kodama Wellness, PLLC, you confirm that:

• You have read and understand this agreement

• You voluntarily consent to receive massage and bodywork services
